Trouble Logging into the Patient Portal?
If you are having trouble logging into your Patient Portal account, please try the following steps first before calling our office.
1. Portal Login Screen- Click "Trouble logging in".
6. You will then receive an email with a link to reset your password.
3. Get assistance with your user name by entering your email address OR your First Name, Last Name and Date of Birth.
2. Select an option and click "Next".
4. After you enter your information you will get an email that displays your username.
5. If you select "Forgot Password" you will be prompted to enter your user name.
8. Enter your new password in both fields and you should then be automatically logged into the portal!
7. When you click the link, it takes you to this window. Enter your user name and answer your Security Question.
